ドクターマリオは、複数色のビタミンカプセルをボトルに投げ込み、同じ色を 4 つ以上並べてウイルスを除去します。このアーケード バージョンでは、NES リリースにあったスロー モードが削除され、各ウイルスの価値が以前のものより 200 ポイントだけ増加するという、あまり寛大ではないスコアリング システムが利用されています。
